Job Title: Locum Physiotherapist - Discharge to Assess Banding: Band 6 Location: Bath Working Hours: Full-time Duration: 3 months Rate: Dependent on experience
Opening Paragraph: We are seeking a dedicated and experienced Band 6 Locum Physiotherapist to join our Discharge to Assess team at The Withywood Centre, Sirona Care and Health CIC in Bath. This full-time role is essential in facilitating the safe and efficient discharge of patients from hospital to their homes or other care settings, ensuring they receive the necessary physiotherapy support during this transition.
Responsibilities: As a Band 6 Locum Physiotherapist - Discharge to Assess you will be responsible for assessing patients’ needs upon discharge, planning and implementing appropriate physiotherapy treatment programs, and monitoring their progress. You will work closely with a multidisciplinary team to ensure seamless care and support for patients during their transition from hospital to home or other care settings. Your role will involve providing individualized therapy sessions, adjusting treatment plans as necessary, and educating patients and their families about their care and rehabilitation. Your expertise will significantly contribute to the overall well-being and recovery of our patients.
Qualifications and Experiences:
Degree in Physiotherapy
Registration with the Health and Care Professions Council (HCPC)
Experience in discharge planning and assessment
Strong understanding of the Discharge to Assess model
Excellent communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
Commitment to ongoing professional development and learning
