Job Overview
Pulse is currently seeking an experienced Band 7 Specialist Pharmacist to work within Older Persons Medicine for the pharmacy team in Portsmouth. The role will be working with one of the largest hospitals on the South Coast and offers the opportunity to provide specialist clinical pharmacy services to older patients within a progressive NHS Trust renowned for research, education, and training.
The successful candidate will work closely with multidisciplinary teams to optimise medicines management, improve patient outcomes, and support high-quality care for older people with complex healthcare needs.
Key Responsibilities
Deliver specialist clinical pharmacy services to older persons medicine wards.
Provide expert advice on medicines optimisation, polypharmacy, and complex prescribing.
Support safe and effective discharge planning and medicines reconciliation.
Collaborate with multidisciplinary teams to improve patient outcomes.
Contribute to clinical governance, audit, service development, and staff education initiatives.
Qualifications and Experience
Registered with the General Pharmaceutical Council (GPhC).
Previous NHS hospital experience working at Band 7 level or equivalent.
Strong clinical knowledge of older persons medicine and medicines optimisation.
Experience managing complex pharmaceutical care needs and multidisciplinary working.
Excellent communication, leadership, and organisational skills.
