Band 6 Occupational Therapist – Community Services
Location: Bristol Contract: Temporary Hours: 4-5 days per week totaling 37.5 hours
Our client is seeking an experienced Band 6 Occupational Therapist to join their team in Bristol. This role involves working across acute and community settings, supporting patients with complex needs and ensuring safe, timely discharges.
Key Responsibilities:
Support discharge planning , prevent admissions, and manage frailty cases.
Provide interventions for patients with falls risk, neuro, and orthopaedic needs .
Work collaboratively with the multidisciplinary team to deliver patient-centered care.
Promote independence and functional recovery through evidence-based practice.
Requirements:
HCPC registration as an Occupational Therapist.
Proven experience in acute and/or community settings.
Valid driving licence and access to a car (essential for community work).
Strong understanding of discharge pathways and admission avoidance.
Excellent communication and teamwork skills.
