Banding: Band 7 Location: Hammersmith Site Start Date: 15/01/26 Duration: 3–4 months Working Hours: Full‑time, Monday–Friday, 9:00am–5:00pm (7 hours per day) Hourly Rate: £23-£30
Job Overview
We are currently seeking an experienced Band 7 Clinical Pharmacist to join the team at the Hammersmith site on a full‑time basis. This is a fantastic opportunity for a dynamic pharmacist with critical care or cardiology experience to provide high‑quality clinical pharmacy services across the hospital.
The successful candidate will deliver patient‑centred pharmaceutical care, work closely with multidisciplinary teams, and contribute to safe, effective medicines optimisation.
Key Responsibilities
Provide day‑to‑day clinical pharmacy services across allocated wards, with a focus on critical care and/or cardiology.
Deliver medicines reconciliation, clinical screening and optimisation to improve patient outcomes.
Support safe and effective prescribing through clinical advice and therapeutic recommendations.
Participate in multidisciplinary ward rounds and liaise with medical and nursing staff.
Ensure compliance with local and national guidelines for medicines management.
Contribute to discharge planning, ensuring accurate and timely provision of medicines.
Promote antimicrobial stewardship and safe prescribing practices.
Support junior staff and participate in departmental learning and development activities.
Skills & Experience Required
Registered Pharmacist with the GPhC .
Essential: Experience in critical care or cardiology .
Previous acute hospital clinical experience desirable.
Strong communication and multidisciplinary teamwork skills.
Ability to work independently and manage clinical priorities effectively.
