We’re looking for an experienced Primary Care Nurse or Nurse Practitioner to join us as a Care Navigator
- a pivotal role at the heart of our service.
The Role
This is not a traditional nursing role.
As a Care Navigator, you’ll be the first point of contact for patients, combining clinical expertise with triage, coordination, and relationship-building. You’ll guide patients through their healthcare journey, ensuring they feel supported and understood at every step.
You’ll work closely with GPs and senior clinicians, while helping shape how a modern, digitally enabled practice operates.
Key Responsibilities
Patient Care & Navigation
Act as primary contact, delivering a personalised patient experience Triage queries, assess risk, and escalate appropriately Coordinate appointments, referrals, tests, and follow-ups Support patients navigating NHS and private pathways Provide continuity, reassurance, and advocacy
Clinical Coordination
Work alongside clinicians to deliver safe, joined-up care Escalate clinical concerns appropriately Maintain accurate records in line with CQC and GDPR
Operational Development
Help design care pathways and service models Engage with and improve digital/AI-enabled systems Support workflow development in a growing startup
What We’re Looking For
Essential
Registered Nurse or Nurse Practitioner (or equivalent AHP) 5–10 years’ experience, ideally in primary care Strong triage and clinical assessment skills Patient-centred, empathetic approach Excellent communication and relationship-building ability Experience in NHS primary care and/or private settings Understanding of CQC, confidentiality, and GDPR Comfortable using digital tools Able to balance clinical and administrative work
