Are you an experienced Occupational Therapist looking to work at the forefront of patient care? Vivid Healthcare are recruiting for an Early Intervention / Front Door service, where you’ll play a key role in preventing unnecessary admissions and supporting safe, timely discharges.
The Role:
You’ll be based within a fast-paced front door team, working closely with A&E and acute services to assess patients at the earliest point of contact. Your focus will be on rapid assessment, functional ability, and discharge planning to ensure patients receive the right care, in the right place, at the right time.
Key Responsibilities:
Undertake rapid functional assessments within A&E / acute front door settings Support admission avoidance and facilitate safe discharges Work collaboratively within a multidisciplinary team (MDT) Provide equipment, adaptations, and onward referrals as required Manage a varied caseload in a fast-paced environment
Requirements:
Qualified Occupational Therapist with HCPC registration Previous experience within acute, A&E, or community settings Strong assessment and discharge planning skills Ability to work independently and as part of an MDT Excellent communication and decision-making skills
