Are you an experienced Occupational Therapist looking for your next locum opportunity within a fast-paced community setting?
We are currently recruiting for a Locum Occupational Therapist to join a Discharge to Assess (D2A) service in Cheshire. This is a rewarding opportunity to support patients transitioning from hospital to home, helping to facilitate safe and timely discharges while promoting independence.
Key Responsibilities:
Complete functional assessments in patients’ homes following hospital discharge Support safe discharge pathways and reduce hospital length of stay Recommend and arrange equipment and minor adaptations Work closely with MDT colleagues including Physios, Social Workers, and community teams Develop patient-centred care plans to maximise independence
Requirements:
HCPC registered Occupational Therapist Experience within community, discharge to assess, or intermediate care settings Strong understanding of discharge planning and community rehabilitation Ability to work independently and manage a varied caseload Full UK driving licence and access to a vehicle (preferred)
